Lumpectomy and Sentinal node biopsy

Hi all, 

I was just wondering if the pathology that comes back after lumpectomy and sentinal node biopsy changed a lot from the pathology information that came through via the needle biopsy. 
I was told I had a small early invasive ductal carcinoma ER and PR positive and HER2 negative. My mind is going into overdrive waiting on the pathology coming back from the surgery, worrying that things may get worse than I'd initially thought. 
I have read a lot of stories where things seemed to change for women after surgery usually with tumours being bigger or having been more aggressive than first thought. 
Id love to hear from anyone who has passed through this stage and how things went.

  • Hi, jjet I am 8 weeks post op. I was only told how small lump was & that lymph node was negative as was clearance of lump but continuing on to radiotherapy. I have asked how high ki level & ca 125 but unfortunately  my surgeon does not do this test. I have a 2 month wait as radiotherapy very busy. I do worry that in time & my age 66 will not be suitable for further treatment. All we can do is trust the professionals. Exercise & eat healthily. I believe the pressures on NHS so great they only do the necessary. We are relying on the hormone inhibitor to keep me here for 5- 10 years. Hope this helps wish you luck 

  • Thanks Pipestril. I appreciate your response. That's quite a wait for radiotherapy but I'm glad your margins were clear and also your lymph nodes. Hopefully you will be well healed for when your radiotherapy starts. 
    I am 46 I got the information in my original post at the time of my biopsy, I'm more worried now waiting for the pathology from the lumpectomy and nodes... I guess just another stage that has to be passed through on this journey.

    You have posted on the ask the nurses section of the forum but anyone is welcome to post on here so hopefully others will share their stories with you. However everyone is different and just because something went one way with one person doesn't mean the same will happen for you.

    I am sorry to hear about your recent surgery and appreciate waiting for the results can be a rather anxious time. 

    Most of the time the results from the surgery do match with what was expected prior to the op but sometimes there are some surprises that can throw people off course and change what needs to happen next in terms of treatment.

    Try not to over think this for now and take it one stride at a time and wait and see what your surgeon has to say.

    My biopsy and initial US results were 15mm grade 1 tumour but histology was 19mm and grade 2. It didn't change anything regarding my treatment and I'm fine now. I had clear margins and lymph nodes were fine. Things do occasionally change but not often, I'm sure you'll be fine.

  • Thanks so much for taking the time to reply Jane. I appreciate it. It's good to hear the pathology after surgery didn't alter your course of treatment and that your fine now. I just seemed to be hearing so many stories of things changing (significantly and for the worse) when people received their pathology results post op and I think I just started to spiral.
    Fingers crossed there won't be any surprises.

    mine did change unfortunately I had a trace in one sentil node then had to have another operation which was to remove 28 nodes and wait again then everything came back clear I to was her2 negative hope this helps good luck

    I was exactly the same last year.  I think it's quite common to worry about this.  My diagnosis was the same as yours and the original biopsies showed an absolutely tiny 1mm of invasive ductal.  I remember my consultant telling me she was amazed it was found and that it may have been sucked out in the biopsy.  Post mastectomy surgery in March, the pathology report showed that it was bigger at 10mm.  I wasn't surprised at this.  Thankfully grade and stage didn't change from the original biopsies and that's important.

    They also found LCIS which was a bit of a surprise.  It's not lobular cancer but it may make me slightly higher risk. So again useful information.

    There were a few changes post surgery but they were incidental findings and it didn't change my treatment plan.  I think it's quite rare to find major changes in grading etc from the original biopsies.  Like anything, it can happen, but it's unlikely.

  • ️ Thanks Mary, yes the waiting is so tough, thanks for sharing the info re your own experiences. I'm feeling a bit calmer after having a couple of days where I was really down and all over the place. My logical brain and my emotional brain seem to be constantly battling these days.
