Double meds and still in pain

Hay so about a year ago I had the worst pain in my right ovary I was taking paracetamol and ibrofen and was still doubled up in pain on the 3rd day I went docs they did bloods an ca-125 came bk at 49, had scan found cysts said it was pcos. I have fibroid's and endos as well thich were found a few yrs ago. So pcos was decided is what is causing pain. And it might be but now every few months just before I start I get this pain only in my right ovary it feels like maybe a cyst might be rupturing. Its so bad I take co-codamol and ibrofen and I'm still rocking and crying In  pain, it goes down my right leg and if I'm out when it happens I limp because of the pain. 

Now I know this could very well be a case of ruptured cyst but I also have other symptoms. My whole stomach is descended I look pregnant. My periods smell like death like a steak that's been left in the sun for week, it been like that for 2 or three years, yes I tell the doctors every time go they ignore me yes its different to how it used to be and yes I know how normal periods should smell, but this is another level and yes I've had BV tests. It's so bad that within 2 hrs of washing I can smell it though my clothes when moving about.

I get heart burn not all the time but enough  that I know it's more than I ever had before which was hardly ever,  like five times my whole life. Also I get a stabbing pain in my tummy. Not sure that's related though. I also need to pee a lot

So went back to docs recently and she asked for bloods again and seemed genuinely keen to help (which is unusual for me) my CA-125 was back down to 19 which is great but I'm still not convinced it's just pcos. 

So I guess my question/s are can the ca125 go up and down if it is OV? And can an internal scan miss cancer or can cancer be mistaken for cysts? 

I also have been very dizzy and feel like my blood pressure has been dropping a lot lately I have to crouch down to try and normalise it and I've been getting very hot a sweaty when doing light activity like walking to the school.

Should I ask for an MRI just to be sure? Has anyone been fobbed of with other diagnosis and it been OV?

    I am sorry to hear about the pain and symptoms you are experiencing, it sounds like you are having a tough time of it.

    Unfortunately no one here can diagnose you or say what the matter is and we always suggest people go back to see their doctor with ongoing or worsening symptoms.

    It does sound like you have a lot going on and having fibroids, endometriosis as well as pcos so don't underestimate these conditions and the chronic symptoms they often cause. Pain, heavy bleeding, needing to pass water more often are all symptoms of these so may be linked to some of the ailments you are experiencing.

    The heart burn and stomach pain is likely to be unrelated but do mention this to your GP if you haven't already. You may also wish to take a note of the foods that may aggrevate or make this worse to see if there are ways of self managing this. Your doctor may wish to check for H-Pylori here and/or prescribe some anti acid medication to try to help settle things.

    Ultrasounds are usually effective at picking up abnormality in the female organs and images assessed by trained radiologists who are trained to pick up suspicious areas that may need further tests or treatment.

    Ca125 is often raised when inflammation is in the pelvis so things such as fibroids or endometriosis often increase this level as well as ovarian cancer. Levels can fluctuate which is why on it's own it isn't the most reliable test and when levels are elevated an ultrasound is carried out.

    Try and chat these things through with your doctor again and say how it is affecting your everyday life, they may think about referring you to a gynaecologist to assess the conditions you have been diagnosed with and to reassess what may be happening. You can't really request an MRI and the need for tests need to be made by the doctor assessing you and your symptoms.
