When someone has been discharged back to care of GP and has developed co-morbidities, can community pharmacist say certain medications are no longer cost-effective and patient should not be on them? What exactly is the role of a practice pharmacist?

Hello there
Practice pharmacists are highly trained, they carry out numerous roles but are also able to carry out medication reviews with patients.
They liaise and work alongside other health professionals who are involved in a patients care to manage a patient and give the best possible treatment and outcome.
As others have said on here they are a common part of GP practices today and whilst they may make some alterations to medications these will be done in your best interests and discussed with you.
I hope this helps
Best wishes
Naomi
Thank you for explaining this, Naomi.
Kind regards,
Rose
Hi – I just wanted to update to say that the review was very thorough and considerably longer than the customary GP review. There will likely be a change of medication, but first an urgent referral to the nurse (within 24 hours). The nurse will do some further tests, may change the medication and will decide whether to refer to GP also. Something has been picked up on.
See Rose, change is good!!
Yes, Carol. The pharmacist asked lots of questions, didn't gloss over things. I have to admit you were right! x
Of course I was!! Replied to the message you sent me. Xx
Hello RoseStarBlue
Thanks for the update, good to hear the appointment went so well and hope all works out okay with the nurse.
You know where we are if you have any more questions
All the best
Naomi
Thank you, Naomi. x
Thank you, Carol. x
Nurse has doubled meds so definitely not cost-cutting!
Also, it has been noticed there was an abnormal liver function result from blood test months ago which had somehow got overlooked. Apparently, doctors still catching up after Covid.
So, now there is an appointment with the GP this week too.
Best wishes,
Rose x
