DIEP FLAP surgery - abdominal tugging 16 months post-op

I had a left breast mastectomy and reconstruction, using tummy tissue, in October 2020. I was slim so the closure was very tight but I could walk upright after around 8 or 10 days and I did all the post-op physio recommended. I've kept asking about the abdominal tightness/tugging, around the permanent stitches the surgeon added to the muscle fascia (below, and to the right of, the scar), and have been told that because I was slim, fit, and had never had children, that it could take 18 months to 2 years to ease off.

But it seems to be getting 'tighter' (16 months post-op) - to the point where I'm aware of it all the time now, not only when I move around or turn over in bed. And it seems to be having an effect on my back and making that ache. Maybe because I'm compensating by using my back more because of the abdominal discomfort? Or maybe because the tightness in the abdomen is stressing other muscles? Or could it be scar tissue?

I saw my surgeon last in Oct 2021 and I could ask to see her again, maybe ask for an MRI to check everything is okay. Or maybe see a physio for some targeted exercises. I run a couple of times a week and do lots of stretching after each one already.

But hearing from someone who's familiar with this would be helpful/encouraging.

Thank you.

  • Hello and thanks for your post,

I am so sorry to hear about the symptoms that you are experiencing despite all the exercises and stretches that you are doing. I can imagine that it must be so frustrating to be left with a lot of discomfort after your surgery.

Because a portion of tummy tissue is removed and transplanted to the chest wall during this procedure, most patients experience some degree of abdominal tightness following surgery as the abdominal skin stretches to the new normal. Being slim does make the recovery process longer and for the scar tissue to recover.

I would go and have a chat with your GP to get them to assess your symptoms with a view to perhaps seeing the surgeon if the symptoms don't settle down.

It would be helpful to get a physio's opinion about where the pain is coming from and whether it is indeed still scar tissue. They may be able to assess whether you need to be seen by the surgeon again or whether there are any exercises that may help relieve your pain.

Simple steps such as changing to a firmer mattress or using an extra pillow to help support you whilst you sleep may help particularly as this discomfort is hurting your back.

You have posted in ask the nurses if you post in another thread you may find that you have more posts. the charity Breast Cancer Now have a service called 'someone Like Me' where they can try and put you in touch with similar women who have been through similar surgery. Have a look here

I do hope that as time goes on your symptoms will improve.

If you feel that it would be helpful to chat to one of the nurses on the helpline then do get back to us or give us a ring.  The number to call is Freephone 0808 800 4040 and the lines are open from 9am till 5pm Monday to Friday.
