Subungual Melanoma

Can't help myself worrying about a brown mark I have developed under one of my big toe nails. I first noticed it over 2 months ago and although it has grown slightly, it hasn't moved up the nail with growth. In the last few weeks I've started getting the odd pain here and there in my actual toe (not the nail itself). I have booked a gp appointment but couldn't get one until the week after next and now driving myself mad browsing the internet like some sort of self-inflicted torture :cry: :laugh:

was hoping to upload a photo to see if anyone has had similar but doesn't look like you can add photos. It's not a straight line but more of a random uneven 'splodge' 

  • Hi, to my understanding if new nail grown without the discoloration on it, then it's more due to trauma. I have a vertical line on my fingernail, had it for about a year now. Didn't think nothing of it and thought it would just go away by itself, it is still there and has got slightly darker. I have been to my GP today and been referred to a dermatologist on a quick 2 week referral. I am so so scared that it's subgunual melanoma. I haven't even told my husband or any of my family. I have had lots of health issues lately and do not want to add anything else to it for them. Obviously on the toe, you could have banged it and I know it can some times take months to heal. I hope you get into your GP and they can reassure you. Keep us updated. 

  • I'm sure you're right - I don't recall any kind of bang or trauma but like you say, it's easily done on your toes. Just found it odd that it hasn't moved for over 2 months. I will update on here once I've had the appointment though. 
    Sorry to hear you're going through that stress and uncertainty. Good that you've got the quick referral though (although I'm sure it will feel like forever while you wait!). Let us know how you get on - will be keeping everything crossed for you. 

  • I managed to speak with doctor this Monday and got a referral to see dermatologist on Wednesday - he didn't seem overly concerned about it being a melanoma but is booking me in for a nail removal and biopsy just to be on the safe side. So now I'm waiting for an appointment to come through, although they weren't able to give any indication on time frame, so I have no idea if it will be a matter of days, weeks or months...? The idea of having my toenail removed is pretty grim but I'm hoping it will be sooner rather than later!

    No one seems to be interacting with my post but figured I'd post updates anyway in case anyone comes across it in the future. 
     

    I spoke to my GP last Monday, who referred me to dermatologist who I saw on that Wednesday. I then had nail removal and biopsy this Wednesday. The actual procedure went well - the anaesthetic injections felt like any kind of injection, so no major pain there. The toe was 100% numb after that and I didn't feel a thing during the procedure. I made a point of looking away but I did have a bit of a sweaty, nauseous few minutes just knowing what they were doing 2 stitches and the a dressing applied and was then free to go - all in less than an hour. Told to keep foot elevated and dressing on until Saturday, when I can then soak it off. Anaesthetic lasted longer than I expected (about 7-8 hours)..... and then the pain!!! It was far worse than I expected and paracetamol alone definitely didn't help, so have been using co-codamol. Still very painful today and I definitely know when painkillers are wearing off! Better than yesterday though, so hopefully will sleep better tonight and improve more tomorrow. Not sure if this is normal or if I just have a rubbish pain threshold ‍♀️
    Now to wait the 3-4 weeks for results...

    Did you go through the same biopsy process before you were diagnosed? 

  • Mine wasn't subungual but was a mole on my leg (superficial spreading). The biopsy process is the same though, except that subungual necessitates the nail removing to access the mark/mole. If the results are positive for melanoma you will need further surgery which is the same as for all melanoma. This is a Wide Local Excision where further tissue is removed to ensure clear margins. Unfortunately with subungual being on a toe or finger, there isn't a lot of tissue to remove so it sometimes means the tip of the toe or finger has to be removed.

    I'm actually not too concerned about it being cancer - I know it's pretty unlikely given how rare it is and even if it does turn out to be a melanoma, I will have caught it early on. 
    I just never expected this biopsy part to hurt so much and for this long! 

  • It will be more painful because of it's location I'm afraid. Hopefully it will settle down soon with lots of elevation!

  • What happened with your treatment - did you catch it early enough for the wide local exsition to get it all?
